OUR SERVICES

The Lambeth Together Training and Development Hub CIC (Community Interest Company) delivers the following functions:

  • Workforce planning - to support the understanding, co-ordination and realisation of health and wider social care workforce
  • To develop and manage – existing/expansion of placement capacity to create innovative and high-quality clinical placements for all learners, to meet the workforce needs.

The Workforce Programme Lead and the Programme Support Analyst are two new roles imbedded in primary care and the support and retention of the primary care workforce, including promotion of primary care as a final destination.
The CIC Board is made up of Primary Care Networks (PCNs), Clinical Directors (CDs), Local Medical Committee (LMC) and Federation Leads while the Hub Steering Group consists of CDs, LMC, ICB, Federation and CIC Working Group.
The CIC are providing a legitimate operating framework, accountability and responsibility for delivery of the training hub education and functions as well as quality assurance. It will have close ties to the Flexible Workforce.

Information is key to NHS. It is passed on from patients to healthcare practitioners and from one healthcare practitioner to another who can use it to diagnose and treat patients. This information gets coded and standardised and becomes data.
Intelligent data analysis results in improved patient outcomes and experience. It also helps in optimal use of resources. NHS organisations such as GP practices, social care providers or hospitals are huge information repositories.
It is a moral duty of healthcare practitioners to use healthcare data for the good of patients, the NHS and wider society to increase the quality of care. Data analytics can help the NHS recognise patterns and trends from the information provided.
This will allow clear identification of areas, services and treatments the require areas of improvement or adaptation. This will only improve the services of NHS, the lives of its patients, improved staffing, reducing costs and waste management.
It will also help in research and funding for further technological enhancement in patient care.

The Finance Department is responsible for the management of the organisation’s cashflow ensuring there are enough funds available to meet the day-to-day payments, including reporting budgeting and Compliance.

We run 2 payments each month, the mid of month and end of month. Cut of for the mid of month is the 11th of each month if the 11th falls on a weekend then the Friday before the 11th. End of month Cut off is the 24th of each month.

The Federation through the flexible workforce is turning Lambeth into a talent attraction magnet, ensuring it is both locally and nationally renowned to be the place to work. It will support practices with both clinical and non-clinical capacity. Staff can sign up to work across the Borough on a temporary or permanent basis which is managed through one point of contact, the Federation. This ensures that the correct staff with the right skills get to where they need in the Borough and no more cancelled clinics.

To review, implement and manage a compliance framework of actions to meet the information governance (IG) standards required across the organisation enabling Lambeth Healthcare to meet its legal and statutory requirements relating to information and data governance including Data protection Act, Data Security and Protection Toolkit (DSPT), NHS and Information Commissioner’s Office (ICO) guidance and CQC. The Federation IG lead ensures that organisation maintains a high degree of awareness and application of IG standards, providing guidance and support to enquiries concerning data protection and confidentiality. Overseeing staff to deliver on their IG responsibilities, monitoring through audits, assessments, meetings, action plans, and offering expert advice on all IG-related issues.

The Federation has developed close working relationships with South East London IG colleagues and work collaboratively in improving IG and building on the use of new initiatives and technology.

Who are we? 

Lambeth GP Access Hubs were established, initially, through Prime Minister’s Challenge Fund money for a pilot scheme to improve access to primary care for Lambeth patients.  The scheme ran successfully, and the Hubs have been commissioned by Lambeth CCG through Lambeth GP Federations through to 2022.  The ethos of improving access remains the same. 

Why are we here? 

We know that patients can sometimes struggle to get a convenient appointment with their GP or Practice Nurse, demand is high and continues to rise year-on-year.  Lambeth Hubs aims to provide extra capacity and choice in the local area for primary care, whilst still having access to the full registered GP records (so we are different to a walk-in centre, out of hours service, emergency department etc).  

Who do we see? 

Patients registered with a Lambeth GP, in pre-booked appointments.  The majority of our appointments are booked on the day by the patient’s GP surgery, some appointments up to 1-2 weeks in advance (but this is mainly for nurses). 

When are we open? 

Monday to Friday 11:40am to 20:00pm and Saturday and Sunday 08:00am-20:00pm.

Where are we? 

We are based within 3 Lambeth GP practices in Oval, Streatham, Stockwell, and West Dulwich. The location of our host practices gives our patients across Lambeth a better access our extended GP appointment.

What to expect?

Patients with an appointment at one of our host practices will still be asked to wait upon their arrival to be seen by the clinician. Although all our clinicians strive to be on time there are a few instants where that might not be the case due to various serious.

Patients are still required to wear face-covering when attending their face-to-face appointment.

NHS Health Checks are done in GP surgeries and local pharmacies. You can have an NHS Health Check if you're aged 40 to 74 and you have not had a stroke, or do not already have a pre-existing health condition.

Your NHS Health Check will be done by a healthcare professional. This will usually be a nurse, but it could also be a doctor, pharmacist or healthcare assistant. The check takes about 20 to 30 minutes. There will be questions, measurements and tests.

 

You'll be asked some simple questions, including:

  • whether any of your close relatives have had the illnesses being checked for
  • if and how much you smoke
  • how much alcohol you drink, if any
  • how much physical activity you do

Your weight and height will be measured to work out if you are a healthy weight for your height.

Your waist may also be measured using a tape measure.

Your age, gender and ethnicity will be recorded.

Your blood pressure will be taken using a cuff fitted over your upper arm.

You will either have a blood test before or at the check. You may also have a small sample of blood taken from your finger during the appointment to check your cholesterol level and possibly also your blood sugar level.

The clinic covers the following procedures:

  • The fitting or replacement of an intrauterine device(IUD) - a small contraceptive device that is put into the uterus (womb) to prevent pregnancy. For the IUD to be effective, it’ll need to be fitted no more than five days after unprotected sex, or no more than five days after the earliest time you could have ovulated
  • The fitting or removal of a subdermal implant - is a device placed underneath the skin and provides effective contraception for a minimum of 3 years.

Lambeth Healthcare Practice is our GP surgery dedicated to providing care for care home residents across Lambeth. We work closely with the care home team to make sure residents are receiving excellent medical care from our team of doctors, nurses and pharmacists.

This work is provided in conjunction with the staff at the different care homes, this helps to maximise patient care needs and reduce unnecessary patient admissions to the hospital, this is done by proactively engaging with residents of long-term conditions.

We also work closely with local pharmacies in providing appropriate treatment plans, making sure all resident have received their vaccinations such as Covid boosters, flu and shingles

Online consultations allow patients to submit their symptoms or make requests to their GP electronically. It is a widely used online digital tool built by NHS GPs. It is designed to enhance patient access, improve practice efficiency and minimise medical care delay.
It takes GPs approximately 4-5 minutes to process an e-consult.  The latest research shows that more than 70% of requests are closed without the need for face-to-face appointments. The Clinical Governance Board ensures a robust system is applied to
requests to ensure the highest security standards are met. Ensuring patient data is always secure.

Lambeth GP Federation understands that the role of the patient is no longer as a passive recipient of care. Presently, healthcare professionals are tasked with actively involving patients in their own health management, treatment, and care.
Furthermore, there exists a multitude of endeavors aimed at fostering patient participation in the conceptualisation, strategising, and execution of healthcare services.
One noteworthy initiative in this regard involves conducting patient surveys, a practice in which the Federation has previously demonstrated success.

Protected Learning Time (PLT) allows GP practices to close for half a day to carry out staff training for the whole practice team. The Lambeth GP Federation has been commissioned to provide clinical cover for all GP Practices in Lambeth during their PLT sessions. This allows the continuity of care for patient registered with a Lambeth GP Practice.

The Lambeth GP Federation offers medical services cover between 12:30-18:30 during PLT dates, this cover consists of reception and administration cover to handle calls and any back-office functions and on the day telephone consultations to patients and if clinically appropriate a full Home Visiting Service during the PLT session.

The Federation has made substantial investments in population health and analytics tools. Our E Z Analytic tools are instrumental in assisting practices in meeting contractual obligations and effectively achieving a range of targets while providing
a transparent overview of their accomplishments through the applications.
Furthermore, the Dr Foster Population Health tools are invaluable resources for practices and PCNs when it comes to identifying and evaluating critical patient outcomes within their geographical area.
These tools are intentionally designed to be user-friendly yet sufficiently flexible, enabling users to efficiently concentrate on patients of specific interest.

The remote monitoring hub aims to alleviate significant pressures on primary care by actively altering and revolutionizing the way patient care is delivered. Remote monitoring allows for the safe collection of data for the transmission of significant factors such as blood pressure, heart rate, and temperature readings to healthcare providers, and enabling clinicians to evaluate and respond to patients in a timely manner.
The remote monitoring hubs aim to offer proactive and prioritised care for those most in need, offer better patient engagement for non -responders, and offer patients better control of any long-term conditions.

Currently Stood down but ready to be reinstated if required.

The COVID-19 pandemic created pressure and unprecedented ambiguity across our Health and Care system. Primary Care was asked to respond in the absence of a full understanding of the virus and robust data on which to base service provision. 

The Respiratory Assessment Clinic was initially set up in response to the COVID-19 pandemic. GP Practices in Lambeth refer their patients who needed to see a clinician but had tested positive for COVID-19 or had long term respiratory problems.

Our Respiratory Assessment Clinic offered a combination of face-to-face appointments and a home visiting service. Most of our appointments were booked on the day by the patient’s GP Practice.

Prior to their appointment patient’s were asked to carry out a lateral flow test, if they were unable to, the patients were provided with one upon arrival at the clinic.

All patients attending the Respiratory Assessment clinic were required to wear a face mask/covering and gloves. These are provided on site.

Service improvement relates to active changes that lead to better patient outcomes, system performance and better professional development.

We employ Service Improvement Facilitators to work with practices on local targets and quality improvement.  Targets are set for general practice to make sure they are meeting our patient demographic needs.
The data is looked at to make sure we are meeting these targets and if not, we can investigate why. Plans will be laid out to achieve these targets to further improve patient care. The targets are based on things like chronic diseases like hypertension. The targets ensure Practices are checking patients’ blood pressure and taking blood tests to make sure the medication is working and isn't isn’t harmful to the patient. These targets ensure GP surgeries are doing all they can to ensure all patient groups are receiving the best patient care.

Lambeth Healthcare LTD. uses a shared record system called the London Care Record. The London Care Record is a secure view of your health and care information and lets health and care professionals involved in your care see important details about your health when and where they need them. Having a single, secure view of your information helps speed up communication between care professionals across London, improves the safety of care and can save lives.   London Care Record can only be lawfully looked at by staff who are directly involved in your care. Your information isn’t available to anyone who doesn’t need it to provide treatment, care and support to you. Your details are kept safe and won’t be made public, passed on to a third party who is not directly involved in your care, used for advertising or sold.
