The Lambeth Clinical Effectiveness Group (LCEG) has been developed by the GP Federation with grant funding from Guys and St Thomas’ Charity. The LCEG includes representation from Lambeth GP Federation, Primary Care Networks, NHS Lambeth CCG, Public Health, Lambeth Together Leadership and the Patient Participant Group Network.
The Group is improving health outcomes of patients by changing the way staff teams in GP practice do things. Lambeth patients will benefit from GPs working differently utilising technology and data, which have not been available to them, until now.
LCEG will make sure that every patient in Lambeth, no matter where they live, will receive the same level of world class health care.
There are over 17,000 people with diabetes in Lambeth and we see more people diagnosed with type 2 diabetes at a younger age related to the ethnic mix of our local population. There has already been considerate local improvement in diabetes care in Lambeth, however we can still achieve more through LCEG with a consistent approach to diabetes care.
LCEG will allow GP practices to identify people at risk of developing long term conditions such as diabetes, asthma, etc as well as increasing the amount of people that we identify with undiagnosed diabetes.
LCEG will use diabetes as a prototype to produce similar improvements on all different aspects of health. If you would like to find out more on the work LCEG is doing please follow us on Twitter @LCEG_Lambeth.
Primary Care Navigators are non-clinical members of the practice team, who are trained to have a good understanding of local services and can help to signpost patients to appropriate services. This navigation means that the GP will have more time available to help patients who most need their skills, such as patients with more complex conditions.

As part of an NHS England Scheme, the Lambeth GP Federations employ 8 full time pharmacists who are in positions across Lambeth.
Clinical pharmacists work as part of the general practice team to improve value and outcomes from medicines and consult with and treat patients directly. This includes providing extra help to manage long-term conditions, advice for those on multiple medicines and better access to health checks. The role is pivotal to improving the quality of care and ensuring patient safety.
Having clinical pharmacists in GP practices means that GPs can focus their skills where they are most needed, for example on diagnosing and treating patients with more complex conditions. This helps GPs to manage the demands on their time.
The Lambeth GP Federations (LGPF) were commissioned by Lambeth Clinical Commissioning Group, to launch the GP Delivery Framework (GPDF) on the 1st April 2014.
The GPDF is a two-year plan that invests in primary care, enabling practices to support patients to stay healthier for longer, and to access services which reduce variation and inequalities in Lambeth’s patient population.
The key areas of priority set for Lambeth were:
- Hypertension – To ensure that patients diagnosed with high blood pressure are effectively identified, monitored, and controlled
- Heart Failure – To ensure that patients with Heart Failure are reviewed every 6 months to check the stability/deterioration of their condition and ensure correct use of medication
- Carers Screening – To identify and support carers with their health and wellbeing, and to introduce social and financial services available within the community
- Patient Referrals – To enable effective management of GP referrals to hospital outpatient clinics.
- Local Care Coordination – To provide patients holistic support for their emotional, social, and physical wellbeing.
- Virtual Clinics – To connect General Practitioners and hospital specialists with the aim of improving health outcomes for patients who may not need an actual referral to secondary care. Virtual Clinics are held on a range of topics, including:
- Diabetes
- Child and Adult Respiratory
- Mental Health
The Lambeth GP Federations have been commissioned by the Local Authority to offer Public Health Improvement Services (PHIS). The PHIS aims to improve the following health areas:
NHS Health Checks: Identify patients at risk of vascular diseases such as Stroke, Diabetes, Kidney Disease and Heart Disease then working with these patients to reduce their risk factors, such as high blood pressure, obesity, physical inactivity, smoking, and raised cholesterol in order to prevent development of a vascular disease.
Long Acting Reversable Contraception (LARC): To increase patient awareness of LARC methods available to women to enable them to make an informed choice regarding effective contraceptive use, and reduce the number of unintended pregnancies
Substance Misuse Service: A treatment programme put in place to reduce harm associated with drug and alcohol misuse to individuals, their families, and the wider community by improving access to healthcare treatments and recovery support within General Practice
Smoking Cessation: During 2017/18, GP practices offered a personalised quit smoking programme providing support, medication, and advice to patients who wanted to stop smoking. This service has since been decommissioned by Lambeth Local Authority.
The Strategic Commissioning Framework is a plan to support Primary Care transformation across London. The Lambeth GP Federations are helping this by facilitating the formation of Primary Care Networks across Lambeth. These networks are made of groups of nearby GP practices who are interested in working with each other and the local community more closely to improve the way that care is delivered.
High blood pressure affects nearly 30% of adults across the UK. Yet despite this, 40% of people in the UK with high blood pressure are undiagnosed and untreated, increasing their risk of having a potentially deadly heart attack or stroke. People with high blood pressure are up to three times more likely to develop heart disease or have a stroke.
To combat this issue, the Lambeth GP Federations are working alongside the British Heart Foundation to check the Blood Pressure of patients through a series of ‘Outreach’ sessions at public locations such as the Civic Centre, Pharmacies, and Supermarkets.
